Who qualifies
- You live in Ohio and are eligible for Medicaid (the state applies income and asset limits).
- The state's assessment finds you need a nursing-facility level of care that can be met safely at home.
- Which program fits depends mainly on age and situation: PASSPORT serves adults 60+, the Ohio Home Care Waiver serves adults under 60 and some children, and MyCare Ohio covers people with both Medicare and Medicaid in participating counties.
- Only the state and its assessors decide eligibility — no agency, including ours, can promise you'll qualify.
The steps
- Apply for Medicaid: online at the state's benefits portal, or through your County Department of Job and Family Services.
- Request a waiver screening; the state's area agency or case management team assesses what help you need.
- If eligible, you're enrolled in the program that fits (Ohio Home Care Waiver, PASSPORT, or MyCare).
- Your case manager builds a person-centered service plan: what help, how many hours.
- Choose a home care provider and start care. You can change providers without restarting eligibility.
